Hmmmm - the link you sent me is broken.  Actually, it's pc_form.php not
pc_form/php... but it's still broken.  I'd be glad to give you the info...

About the Viola!  It's my embarassing stab at the French "voila!"  Sorry
for the misunderstanding.
